There are lots of great devices that do this on the market. A few come to mind.
1) Bump for iPhone. Bump another iPhone user and get their contact info. Check it out here: http://www.bumptechnologies.com/
2) There is also MS Tag. You can add it to anything: your email, a direct mail postcard, a billboard, the possibilities are endless. And since you can pre-populate the tag with the data YOU want to share, it's extremely flexible. Demos here: http://www.microsoft.com/tag/
Of course these aren't quite as "cute" as a Poken but....there are some options!
